Record Requests

Attorneys – Record Requests

To process and release medical records, we require one of the following:

  • A completed and signed Authorization for Release of Medical Records, specifically authorizing the release of records to the requesting attorney.
    • This must be done via the Medic’s Authorization form which can be completed by using this link.
  • A subpoena signed by a judge authorizing the release of the records.
  • A court order authorizing the release of the records.
  • A signed records release from an authorized representative of the patient, such as a Health Care Power of Attorney, legal guardian, parent (if the patient is a minor), Executor, Administrator, or Attorney-in-Fact, permitting the records to be provided to the attorney.

Enforcement Agency – Record Requests

State and Federal Agencies such as: Law Enforcement, DHHS, and Department of Labor, may request records here.

Patient and Non-Attorney Record Requests

Complete this form and upload any required documentation.

Required Documents

  • Patients – Valid photo ID
  • Spouse/Next of Kin/Healthcare Power of Attorney – Valid photo ID and Healthcare Power of Attorney, court issued Letter of Administration, or this form notarized.
  • Parent of Minor Child – Valid photo ID, and birth certificate, guardianship paperwork or this form notarized.
  • Guardian – Valid photo ID, and court issued guardianship papers or this form notarized.
  • Executor/Administrator/Attorney in Fact – Valid photo ID, and Executor paperwork, Letter of Administration, or this form notarized.

Processing Time

Once we receive all documentation required under applicable privacy laws, we will begin processing your request.

  • Expedited processing is not available.
  • Requests are processed within 30 business days of the date all required documentation is received.
  • We do not provide attestations.

Additional Information

Records can be picked up at Medic headquarters during business hours. Bring a valid photo ID and any required documentation.

The Records Department does not handle:

  • Itemized statements
  • Payoff requests
  • Balance verifications
  • Medical insurance company records requests
